Case Management is the heart of our client services.  The goal of Case Management is to help connect clients with with the community services they may need in order to stablilize their lives and help them remain in health care.


Once a qualifying client has completed an intake, our Case Managers may help them with a variety of services, including:        

• Access to Care       

• Housing assistance        

• Referrals (Mental Health and Substance Use)       

• Identifying barriers and approaches for transcending barriers        

• Arranging transportation for medical appointments (for those who qualify)           

• Referrals to other services
