Case Management is the heart of SLAC's client services.  Together, SLAC, the Resource & Wellness Center, and the Comprehensive Care Clinic offer both non-medical and medical Case Management. The goal of Case Management is to help connect clients with with the community services they may need in order to stablilize their lives and help them remain in health care.

Once a client has completed an intake with SLAC, our Case Managers may help them with a variety of services, including:        

• Access to Care       

• Housing assistance        

• Referrals (Mental Health and Substance Use)       

• Identifying barriers and approaches for transcending barriers        

• Arranging transportation for medical appointments (for those who qualify)           

• Referrals to other services

A client’s eligibility for SLAC Ryan White Case Management is dependent on the following requirements:        

• an HIV positive diagnosis

• verified Louisiana residency

• federal income requirements

• a SLAC reassessment every six (6) months after initial intake
